I had 18 minutes to speak in in America
it’s up to average time that you would
have to wait
pass your appointment time before you
can hear the documents to you as the
patient sits in the app and called
waiting room she’s worried she’s anxious
she’s looking for answers she knows what
the role of the doctor is but what role
does she play in the doctor-patient
relationship and that’s what I want to
talk to you about the role you play in
the relationship you have will have with
that person who keeps you in your dock
but what a32 I have if any to talk to
you about the doctor-patient
relationship
well I’m a solvable claim festival so
what you may ask is a professional
patient but before I can tell you what
it is I have to tell you what I used to
think it was it used to define it as
someone who had to go see the doctor or
lab product terminal illness this could
be someone who spend a disproportionate
amount of time with my presence
everything came to where that what the
professional relationship is much more
than that
my careers were professionally h2p Emily
was raised by a woman grandmother who
had a lot of medical problems but she
was also a full-fledged afrikanya and as
the very interesting person family I was
the one that took her to the doctor so I
was local translator and witness
qualities occupations correct
but I also have my own set of ourselves
with all the complications that come
with that and when I was fifteen
thousand II tumor to attend Atlanta and
to go and tell my family this awful news
it also had to be go she ate with the
documents whether or not to have the
brain tumor removed at the age of
fifteen imagine I had to make
life-altering decisions that would
compromise my ability to have children
so I never had the tumor removed and I
went off to college
first generation of my family to attend
college and graduated from for no
university lovers I attended Georgetown
University Law Center other prestigious
fellowship it was a lot of you graduated
with one of the top student awards past
New York art and it became a public
defender so I’m just gonna go ahead and
admit it it was well I had a bag of
chips but the whole time I was doing
this I was also really second twenty
years into my legal practice I was
diagnosed with lupus lupus is an immune
deficiency disease basically in layman’s
terms what it is is that my new system
just all the study decided to attack in
my own body and its systemic so with
systemic means is it little be
attractive my whole body I would
actually come to be diagnosed with fire
or immunities work what you mean and as
this attorney speaks to you right now
the professional pollution of me is
undergoing oral chemotherapy and just
this week I had to have procedures to
both my esophagus and her vocal cords
because of complications from the
autoimmune disorders one of my doctors
will put my favorite doctors just up you
know Facebook she said I’m not worried
about you if anybody knows how to solve
problems it’s you
so I want to go back to when I was first
diagnosed with lupus and I was working
as a public defender like a hot thing in
August of 2000 while I was waiting and
one for me to be on the decision to have
the last piece with me for my favorite
food cart I love that food it was my
favorite but as you can probably guess I
got food poisoning and it got so bad
that I had to be admitted pretty pretty
scary right well ended up being anything
but the plan was for the doctors to give
you some people
hypotonic stated basically that this
means it has three times the amount of
sodium of the normal three dose would be
dangerous so they settled right Leonard
before ours except I when I was admitted
the hospital personnel using complicated
medical history they knew they even knew
that I was an attorney
but two entire days passed without
doctors coming to throbbing but I hit
the flipper button
I asked anybody who came in even yelled
out into the hallway
hey is someone gonna draw my blood but I
was told that it was it was the summer
they were understaffed and they could
get deployment like where they could and
I believe me now the second night here I
was at the hospital I was sitting by the
window by herself and I realized that my
body was violently shaking and can taste
the weather my mouth as I was biting the
insides of my cheeks
I couldn’t form a coherent part it
couldn’t even call for help
I was seizing and I was dying and you I
was in like that managed to do was pray
for more time but wasn’t ready to dock
and waited on the doctors and waited on
the nurses but they never hit it splits
into a coma when I woke days later I was
told that I was lucky to be alive
bring them some of the side effects from
the brain injury included seizure
disorder cognitive with family problems
speech impediments severe balance
problems I mean you get the picture I
was a mess and it took six years of
information and publish human look
rehabilitation to get to where it where
I am now and I don’t want I think I look
pretty good so people say that it’s a
miracle that I’m alive so to me the
greater miracle is that I can use my
story
all this horror that my story for all
its horrors is a tale of resilience and
overcoming obstacles the more apartment
I want to use my current being knowledge
to empower current and future patients
after the injury my body was broken and
my soul was crushed the last people I
wanted to see especially what we needed
of the Fluke event for them where the
very people were nearly community great
but what trust Center had so I had to
get over here and then a couple of
months into my rehabilitation I start
thinking what can I have time to
facilitate a better communication
between my selves
would that made a difference yes that
got me to wondering what can patients do
to enhance their communication with
doctors to have a better than occupation
interaction so my surgery disasters
led me to do online research he read
books and talk to the wise people one of
these conversations led me to the
Masters in narrative medicine program at
Columbia University the narrative
medicine is an emerging field that seeks
to trained clinicians
it’s a better fear interpret and be
moved by patients stories adults
basically it’s a movement to improve
healthcare it’s soon after graduating
into the program amita realization in
order to have to make the most part of
the doctor-patient interaction both
parties have to show up as professionals
now I’m not insinuating that well that
the doctor and the patient are equal the
knowledge or skill no inegalitarian
relationship that the medical level is
not possible but but the best results
come when both the doctor and the
patient are hit share concerns and
expectations and they work and think and
sick so I want this leg needs to be
defined with professional patience it’s
not the frequency the appointments for
the nature of occupation experiments
whether the patient’s Nederland when it
comes to their doctor and I found that
there four easy steps that patients
continue they can leave them to have a
more professional attitude and thus more
professional activity will enhance the
doctor-patient reps so I call the four
steps he’ll be paid up pretty quick
so the game stands for honor and or a
new doctor’s time by coming from show
them prepare to visits he stands for BMX
your body and knowing your medical
history he stands for assembling your
own healthcare support team in L stands
for listening to your doctor and
understanding what their sex you my part
of so so let’s go to the first honor
your ed your doctor’s tire so did you
know that a physician spends an average
of 13 to 16 minutes per patient during a
doctor closer
that leaves the patient a very short
amount of time in which to share their
concerns from the data you make the
appointment to the moment you hear the
doctor will see you now
weeks if not once impacts to patient
needs to most make the most of that time
before I give you money how to get
prepared I’m gonna talk to them well
these wonderfully dumb people in the
audience I’m sure a lot of you were
thinking well this is a nice talk to me
doesn’t contain him you may never get
sick I’m here to give you some bad news
you will have to go see a doctor at some
point and if not be yourself to take a
loved one or a family member so you also
have to pay attention to the steps so
who an easy way to get prepared is to do
online research sorry so all the
research will help you become familiar
with medical terminology and it will
help you be able to have a better sense
of your symptoms and a better
conversation with your doctor but I do
have to do a caveat and stay aware take
the true beat on these American websites
will agree this all my husband who never
gets it recently had some symptoms
and I was like I have to get a computer
and find out that’s it and after looking
at these medical websites
I was this close to diagnosing him
against the popular reports that he had
pulled with this golden whip notes but I
say that just to say that you know this
is just to give you some information you
obviously have to discuss what you find
with you doc another way that I prepare
is by going to the my chart online
program so most hospitals and medical
centers have this pie chart online
program this is what it looks like just
in one place and they look different
based on what hospital you go to but the
point is that this has your entire
medical record all you have to do is ask
for the access code to get that
information what you want to do is check
your profile and get your lab results
your latest MRI reports for extra points
in use all of this information to
front-load your questions for the doctor
so when you live in you know actually
what you want to discuss with you not
and the last way that I prepare is by
creating a document that I call a
medical resume so I got the repeat hold
up I like to impress my daughter I like
to come prepared I’m America resume my
food all my biographical information and
I include my diagnosis dates diagnosed
current all medications in just other
relevant medical history this
information is really hard to remember
on the spot and so it’s good to have it
especially at your doctor’s office when
they’re asking you you know well what’s
the medicine today so it’s really good
I found that it impresses the doctor
does that give you that right so the
next step is being an expert in your
body so you know more about invite then
you think you know what crap if you wear
that to it you know what aches and pains
you get or the rains and snows and you
know what food you love to eat and there
always upsets your stomach so another
way to become a greater expertise body
it’s just to pay close attention so
honey body reacts to the environment –
what you mean – what you know this
doesn’t have to be a major undertaking
no need to drive everything down the two
samples to pay close attention let’s say
you have a cold you shift you can ask
yourself how long have I been popping am
i short of breath do I have a plan what
color is it
these are all questions that your
doctors don’t want to know so you want
to have that information record now one
of the reasons one of the obstacles for
patients to talk clearly to doctors is
something that we call the white coats
and that’s the fact that patients see
doctors has thirty figures with little
time to spare so they’re reluctant that
are not scared to interrupt the doctor
asked a question or speak up but we
don’t get over this white coat there to
talk to me miss out on pretty
information in these two diagnosis
recent studies have shown that when a
patient starts telling the doctor what
welcome to see them the doctors going to
interrupt between 12 and 18 seconds more
remarkably soon studies show that if the
patient was left to speak without
interruption they would stop speaking
with just 48 seconds or less that means
is that all it takes is 30 seconds
before you fully share what broccoli
they’re for the doctor to get this
printed information and that’s why I
argue that you just have to get open
this white coat of fear and they need to
embrace their goal in the doctor-patient
partnership because that’s what it
should be a partnership the both parties
should be alive so assembling your own
healthcare team if you think about it
most initiatives have a team of
professionals to collaborate with to
make a diagnosis or treatment it could
include billing and coding for nursing
support colleagues
where’s Richard should you be making
your own healthcare decisions on your
own
well I can tend to note you should have
your own health Fair’s for tea so
ideally you can bring a friend or family
member when you go see that and they
could ask questions take notes and be
there to support you but if this is not
possible as awful is that you should
confer with your team consisting of
Craig’s family members and you can make
the list of there’s sometimes any
questions you want to discuss with your
doctor
basically the point is that you want to
have a group of peers if you trust and
help you
health care decisions so the last two my
tip steps it’s listening to your doctor
and understand what they’re saying so I
just want to say this is my husband’s
favorite that step is he he says many of
us with really guilty of not doing this
right so it’s not enough to nod your
head yes or yes man the doctor you must
demonstrate that you understand what the
doctors saying to you what is this well
I could want to feedback to the doctor
where I’ve heard her say including tests
medicines etc basically I summarized the
encountering in front of the doctor and
this is important because a recent study
showed that more than 50% of doctors
forget to tell patients but the dosage
of the new medicine is for how long they
have to take medicine so the patient has
to speak up and ask for that patient in
this moment of summarization is a good
place for you to interject any questions
or concerns you may have or may have
just been too shy to have got to ask
earlier the point is to have good
etiquette I know it sounds a little Miss
Manners and trust me it works the
doctor-patient relationships work you to
have to your communication and attempted
listening
so I argue that you should always make
sure you hear from the doctor saying in
other words don’t hear her to my ear and
don’t leave the doctor’s office unless
you ensure you understood everything
when he’s discussed so those are my four
steps and if you follow them it does not
mean that your doctor’s going to be your
new BFF yes my god
that’s not to go okay but what it tells
me is that you will be able to bring a
more professional attitude to your
doctor’s visits is that and hopefully
you’ll be able to find a partner a
reliable partner in your healthcare so
being a professional page that doesn’t
mean that you can smooth through your
health so that you’re constantly at
doctors Isles
no quite the opposite it means that
you’re really good at managing your
health and they give it the most part of
every doctor’s visit when you quit
marriages and hopefully you will have
less doctor’s visits so I can’t blessed
to be alive I’m lucky to be a tree who
gets to teach narrative medicine at
Columbia University and CUNY School of
Medicine and I love my my students they
inspire me every day I’m committed to
being I’m committed to standing up for
myself as a patient
I’m committed to speaking to speaking up
to ask you questions I’m committed to
being an active agent and the
doctor/patient partnership especially
now with the impending changes in
healthcare I hope that my story of sort
of how our other patients to embrace
their role as a professional patient and they see them at 18 minutes
it’s up to average time that you would
