Extra-Terrestrial Medical Care | Scott Dulchavsky | TEDxWayneStateU

[Applause]

all right I’d like to take you on a

journey to space so as we say it s up

sit down dragon shut up and let me enjoy

it arrived just after eight minutes of

Indian start you’re in space going

17,000 miles an hour your other two

miles of open planet you have an amazing

vista see the beauty of our planet its

challenges in its limitations the

International Space Station arguably

most challenging and ambitious endeavor

that humankind has never created 16

nations came together with their best

and the brightest the innovative to

solve almost insurmountable challenge

this has contributed not only into human

performance in space for now going over

15 years and also amazing innovations

that have come back for a planet to help

us I’ve been privileged for the last few

decades to work with NASA on this far

medical system scheme and on the human

research project but we’re trying to

develop anonymous devices not only to

improve care for these men and women

above our planet but also bring them

down and I’ll use some of the things

that we’ve learned over these last

year’s try and talk about some of the

challenges that we have today provided

here not only here in our city for the

crosshair globe now might not be

immediately apparent to you the

providing care on the International

Space Station in underserved areas in

our world are very very similar but

Katie came back two days ago to the Fife

and are my current crew medical officer

on the international space station is

now a Russian engineer we had 80 hours

with him to train him on medical

procedures and let me be frank you’re

much more interested in toilet

maintenance because you know the

consequences of the toilet going dead

which it will and that meant the

potential develop development and the

situs through a real limitation in the

medical training under people providing

the care

with a limitation in resources it cost

ten thousand dollars a kilogram the

lamps something into orbit so the

international space station while

amazingly complex as medical equipment

to approximately of an ambulance of food

in our city today certainly our globe

every 90 minutes communication is a

challenge so very often we are not in

direct communication I take you over to

the other Cal that you see above me and

those exact same constraints are seeing

across our world and here at home in

terms of access to high quality medical

care but with these big wicked problems

comes the amazing potential of

innovation off on up there now is you’re

starting to think about the next phase

of exploration class spaceflight below

the long of past low Earth orbit I’ve

got a big problem for Hemi in it let’s

start planning on a trip to Mars so it’s

going to be a three year journey there

are no walmarts on the way so we have to

think ahead for over 500 medical

contingencies that they in fact occur

the medical supplies on us will be

woefully inadequate for many of those we

don’t have Diagnostics right now we

don’t have an x-ray machine we have no

CT scanner and that so we don’t know how

to diagnose live alone treat many of the

conditions that may occur let’s take a

little bit of our communication I’m a

surgeon so if I help accrue three

quarters of the way to Mars hey don’t

cut there they don’t hear that for

almost 15 minutes so the telemedicine is

your practice is today on our planet

really it doesn’t work in space and time

that I’m starting to talk about how do

we provide medical care when help is a

140 million miles away that is the

challenge that were faced with today now

we can we have an ultrasound machine

many of you are familiar with ultrasound

to tell you the sex of your baby whether

you have gallstones perhaps some things

about vascular foot really good to know

whether it worked in outer space or not

my team is not performed over 500 hours

of altars up on the international space

station crew for the shuttle and we

learned a lot of things if you were to

learn altis on here on the planet it’s

about 500 hours of training and I

already told you fear woefully

inadequate training time on these very

private individuals so we came up with

ways to just in time really quickly

teach mathematical people really

complicated medical stuff but we really

didn’t care about pregnancies hopefully

others basically and improve the life

might not work over night we do worry

about these other conditions and so we

had a tool and we started to use it in

different ways and then so now we

utilize ultrasound on a weekly basis on

the international space station to

answer some critical questions many of

those questions have now translated back

to the planet if you go to an emergency

room in our country today with a long

problem no doubt an ultrasound will be

utilized because of our work to diagnose

what you have going on you may have

where we’ve had some problems with

eyesight on their long-duration fruit so

now we look at eyes as an index and a

window into the brain kineton this is

not translated fed to our planet to

looking at brain function utilizing an

ultrasound machine there with another

fork because we didn’t have to on the

planet we have CDs and hours and things

like that really importantly these

devices are portable so they can go

battery-powered anywhere so we’re

starting to look at this global so now

we started to talk to people of it the

United Nations the World Health

Organization working with the Millennial

development project trying to prove

things like infectious disease maternal

care how can we tell whether that

individual can deliver that child in it

in her

hiland versus having to get on a dumpy

go four days to a higher level work here

so it’s now been in over 60 countries

trying to answer things like them we

also look at musculoskeletal problems

when you’re in a zero gravity

environment lose bone and muscle mass we

have a manager there in altered exercise

accordingly so now we can utilize all to

something I just broke it off muscular

problems and things like that so we now

have this amazing tool that individuals

more factors could utilize any pressures

of ultrasound programs now in the

country utilize these techniques that we

develop the number Detroit and we get

this grid technology and I was trying to

figure how many hell kind of wings

tickets out of business dr hurch two

tribe members I said hey we’ve got this

pretty neat technology it did tell you

right on the spot whether or not you

have something bad in your athletes and

so now they’ve incorporated that so if

somebody gets hurt at the job I’m not

only would utilize this technology to

tell your good to go you get back in the

game or you might want to sit up this

next period now apparently athletic

trainers taught and so we were

approached by the united states olympic

committee and they said you know we have

a problem in torino the spirit areas of

high mountain roads and they’re not

accessible to high elevation to flying

helicopters man so could we utilize this

technology on various venues in the

Olympics to try and with athletes on

dolly get hurt to have a quick

assessment of them we performed over 200

examinations if you need spendings now I

thought the money is going to be in the

inverted aerials and hockey and things

like that so I signed up for those men

you you know where people got reason

home figure skating forgotten

and I guess it makes sense with all of

the spinning and things that they go on

my vest which utilize their quite a bit

like that so what subsequently doing

every Olympic being sent providing

point-of-care information to trying to

love the athletes to ascertain whether

the computer so we were in vain over a

couple of years ago Lindsay blonde was

supposed to win for women’s downhill you

might have remembered it three days

before her event she was on a practice

run she caught an edge about 80 miles an

hour she got a vicious still and she was

a evaluated five that take one of the

few positions of the Olympics and I said

yummy silencing to figure out and

Lindsay was a real tough competitor so

she said you know heard about this dude

the thing that were you guys in could

you you can’t look at me so we evaluated

her using this technology she had some

injuries and I said you know we don’t

think it’s going to be food it’s not

hurt like hell but we think it’s not

going to be career with an account for

you so Lindsay’s Skiba days later she

won gold mouth so I consider one

sixteenth of this gold medal vultures I

will never get to converting an Olympic

gold medal now I hope it uh suggested to

you we’ve been able to solve some pretty

tough health problems in a wide variety

of areas we have done ultrasone

examinations on Mount Everest for high

altitude problems that you get in an

extreme environment we’ve certainly been

in a lot of sporting venues we do a lot

of examinations are across the world

trying to define some of these health

problems that heretofore have been

unsolvable and then we don’t have

factors in most areas ninety-five

percent of our locust underserved as it

relates to quality medical care patches

of the doctor level

below and often you’re going three to

five hundred miles for any type of

radiology x-rated intervention or things

like that we started to look at this and

train other individuals who by this high

quality medical care and this shows

somebody performing and well-being

examination in any way this is about 50

miles just south of the North Pole prior

they had applied almost a thousand miles

to get this a type of gear and Canada at

great expense great hardship the duties

of maternal and it’s the tournament

challenged individuals so I think we

move the ball a little bit on potentials

for healthcare across our country I

don’t want to leave you with just saying

that this only solves the problems in

far-flung areas up in space or at the

north pole or Mount Everest or in

sporting venues and things like that

today in our cities across our country

and if we have the same disparities in

health care net so we have to think

differently I can’t throw daggers in

millions of dollars equipment to solve

these health problems that played us

today we have to be thinking differently

so we started to utilize hospital as

factories of innovation now everybody

has dick and every one of the job to get

half be somewhat MacGyver life and

innovative you have to think outside of

the box a little bit but we didn’t have

the seal ways of doing that in hospitals

we’re all about health care we really

couldn’t think about voices this is a

real good kernel of an idea so we can

all of the now five years ago an

innovation Institute at Henry Ford and

this is where we try to solve those big

wicked problems that we have to tackle

every day we have to do things faster

cheaper more safely and that more

effectively more scalable without

throwing resources at it but we also

know that we don’t have all the ideas so

we’ve come up with unusual partnerships

one of those is mobile here we work with

a college or creative

science now this is a design center

organization what they possibly

contribute to a hospital where we

invented their students I have Henry

Ford the first thing they noticed was

looking at our hospital gowns and

they’re gone what that mean kidding me

you’ve been in this a hundred years and

you came up with something that your bum

ain’t up diggity there at all so they

started to innovate once this simplest

of problem if anybody who’s been patient

gets relate to and now they are gone

with dignity and that he is not

commercially viable we’re also use

utilizing global health in this so we’ve

reached out to other countries India for

example where approval innovation they

have to do things for large masses of

individuals very cheaply the State of

Israel which is second to Silicon Valley

and that we now have them innovate with

us solutions to those daunting

healthcare problems that continue to

plague us today now when President

Kennedy said almost 50 years ago that

we’re going to go to the moon in under a

decade there was unprecedented

enthusiasm and ambition that resulted

not only in a successful law banning of

Apollo 11 but innovation that brought

things to us such as miniaturize of the

medical devices hello Madison scratch

resistant glasses water filtration

insulin pumps things like that every

tank for god sakes and things that help

us every day I think that criolla NASA

better faster cheaper it’s never truer

and in medicine the meat than it is

today we have to think differently now I

won’t be one of those men and women that

ultimately sense flip on the surface of

Mars but it’s my hope that I’ll help it

contribute to their medical cure and

route and while they’re on Mars and I

hope that that terror translates to help

all of us here today thank you [Applause]
