Direct Primary Care: Remaking the Health Care System | Shane Purcell | TEDxFurmanU

[Music]
today I want to speak to you about the
credit cycle the citric acid cycle
more specifically the critical third
step in this cycle no I’m joking I don’t
want to talk about that at all I don’t
even remotely want to talk about that in
fact I was on campus yesterday they were
touring me around and I passed by the
Science Building started to get the
willies just thinking about this uni
biology majors out here pre-med majors
you know what I’m talking about you guys
should be out there studying this right
now so there’s no pre-med degree right
no biology degree tough to get in med
school without spending a lot of
sleepless nights studying this little
gem right what about the incredible
story of Inez Ramirez a 40 year old lady
living in a small village in Mexico in a
March of 2000 she was pregnant with her
seventh child went into labor alone at
home 12 hours into labor she realized
this is not working out too well she
feared for her life and the life of her
child
so she did something miraculous
she took a kitchen knife and perform a
c-section on herself
removing the infant from her womb alone
and without anesthesia amazingly as you
can see in the picture most she and her
infant survived and I was a second-year
resident with this occurred and we were
talking about this just amazed one of
the older residents stood up said you
know something really terrible about
this story this woman never spent one
day
in the Krebs cycle and he was right 17
years of practicing medicine not one day
of I used the kreb cycle get to help a
patient so often things are more
complicated than we want it to be health
care is the same way today I want to
share a little bit about how we got this
complicated why it’s so expensive and
some simple things that we’re doing to
change that let’s start right here you
might have seen this is slide before
this is a representation of our awesome
health care system pretty incredible
looks like a nightmare to me is worse
than the Krebs cycle so because of this
bureaucratic nightmare of regulations
red tape and rules we’ve turned our
health care system into something that
separates the physician and patient
relationship now a physician gets about
eight minutes to spend with a patient
and you can waive as as long as four to
six minutes in my community just to get
a visit to see your physician and for
every hour I spend seeing patients I
spend another two hours doing paperwork
computer work or some kind of desk work
it really has nothing to do with patient
care so how do we build relationships
how we get to know our patients how we
get to help them if we don’t have the
time to spend with them in the last 50
years we see that the foundation of
health care is the patient-physician
relationship since the time of
Hippocrates but somewhere in the last 50
years we’ve lost that we’ve forgotten it
or even worse it’s been corrupted I’ve
got some shocking news for you our
health care system is not broken and we
may not be working for me and you but
it’s working exactly the way those in
power want it to work
for decades now our government insurance
companies hospital systems
pharmaceutical industries they ama and
other organizations have spent a lot of
time and energy into building up our
health care system into a three point
three trillion dollar industry that’s
over ten thousand dollars for every
person in America these corrupters that
I call middlemen stand in between what
we do as physicians and and how we help
patients they create rules regulations
bureaucratic red tape to keep us
separated from our patients and they’re
really good at it too they make billions
of dollars doing it and they’re so good
at it then in 2016 these groups
collectively spent five hundred and nine
million dollars lobbying Congress just
themselves more than any other lobbying
group in America that’s one dollar out
of every six spent lobbying done by
healthcare organizations so let me ask
you this but all that power and all that
money flying around you really think
those organizations really want to
change our health care system no they
would desire the status quo the good
news is we don’t have to change our
system because like Bucky fuller said to
change something we built a new model to
make the old model obsolete and across
America physicians like me and others
are building a new model for primary
care called direct primary care drip
primary cares a way to simplify health
care and return it back to a patient
physician relationship between a trusted
physician and open patient we do that by
removing barriers we break down those
red tape all the complexities that
separate the physician and patient
relationship
and when we do that we go from something
really complex like this to something
simple like this and that’s how we get
direct primary care so how does direct
primary care work you have a patient who
pays a physician directly for to access
unlimited services those services
include office visits phone visits
telephone visits video visits and also
access to C to text them or even send
them an email message and those things
come in handy especially on the weekends
or after hours it’s paid out by a
monthly membership fee that can range
from anywhere from $10 to $100 and that
fee includes everything we do in our
clinic so EKGs strep tests urine tests
your pregnancy tests skin biopsies
suturing anything we can do in our
office and include it in that monthly
fee no extra costs no co-pays everything
is done right in our clinic we also
provide labs and generic medications in
our office at about 90% of the discount
price so a lab test like a thyroid test
will cost you two dollars a cholesterol
panel will cost you four dollars some
clinics in town you can spend anywhere
from fifty to two hundred dollars for
one of those tests we also discount our
medications given right to our patients
in the office so blood pressure medicine
like lisinopril would be three dollars
for a 90 day supply or an antibiotic
like augmentin for six dollars and you
don’t have to go to the pharmacy and
wait hours to pick up your medicine
you’re leaving right from our office
with your medication we also have access
to discounted imaging my cat scans MRIs
ultrasounds and x-rays all at discounted
prices so you could say threat primary
care is like the Netflix of health care
and it’s totally done outside of
insurance we don’t accept any insurance
whatsoever so we we cut out the
middleman and when we cut out the
middleman it allows us to lower our
price and increase the amount of time we
spend with patients they get more access
to care instead of eight minute visits
they get 30 minute visits or 45 minute
visits whatever they need patients like
my friend mark
he’s a 58 year old construction worker
no insurance and some serious medical
conditions he was paying over nine
hundred dollars for one of his generic
medications nine hundred dollars a year
we had the same exact medication in our
office for $48 a year saving him enough
money to pay for his entire year of his
membership he was pretty happy the King
family they have great insurance they
had poor access to care they wanted to
be able to see a physician on their
schedule when they wanted to see them
next a same-day visits and they wanted
to be able to access us through the
internet or email or video visits
because they like to travel a lot so
direct primary care is about making
health care simple again somehow in the
last 3040 years we turned RIT routine
health care your family medicine we turn
it in some kind of crazy contraption
it’s complex and it costs a lot of money
and I don’t know why that is but truly
health care routine health care like we
all use is not expensive it’s not
complex as physicians our oath is to
patients not the insurance companies not
to the government and not to
administrators so what we’ve done in
direct primary care is we’ve told those
third party middleman well you know the
guys you know the saying around this
campus right
so as I wrap it up I want to ask you
this who does your physician work for do
they really work for you or the
insurance company or the government at
our office we work for patients our
focus is on patients and the nightmare
is over look around you might find a
direct primary care clinic around you
and help us create a new health care
model from the ground up thank you
